Patient Financial Agreement

This agreement explains the fees, payment responsibilities, and financial terms that apply to services provided by MBR Health. It applies to your current and future services unless withdrawn or replaced. This Financial Agreement forms part of the terms on which MBR Health provides its services and should be read together with our Terms of Use and Privacy Policy, available at www.mbrhealth.com.au.
1. Understanding Our Services
MBR Health provides diagnostic imaging, specialist radiology reporting, image-guided procedures, and interventional treatments. Fees are charged for the professional radiology services provided, including image acquisition, interpretation, reporting, and procedures.
Diagnostic imaging is performed to investigate the clinical question identified by the referring practitioner. A normal, negative, or inconclusive result does not mean the examination was unnecessary, inappropriate, or incorrectly performed. Further investigations or different imaging may still be clinically indicated.
Image-guided procedures and interventions are performed to diagnose, treat, alleviating, or managing medical conditions. Individual outcomes vary and that procedures may provide complete, partial, temporary, or no benefit. No guarantee can be made regarding any specific clinical outcome.
2. Scope Of Services
All diagnostic imaging and interventional procedures are required to have a valid referral. Radiology examinations and reports are prepared in response to the referral, clinical indication, and information available at the time of service. MBR Health may modify, substitute, add to, postpone, or cancel a requested examination or procedure where clinically appropriate or necessary for patient safety.
Unless separately arranged, MBR Health does not undertake comprehensive review of all historical medical records, ongoing specialist management, second opinions, or assessment of new clinical questions. Additional review, reporting, or consultation services will incur additional fees.
3. Fees, Payment & Refunds
Fees are charged for professional radiology services and expertise, and are not contingent upon obtaining a particular diagnosis, finding, treatment response, or clinical outcome. At the time of booking, we will do our best to provide an accurate estimate of costs, including any Medicare rebate entitlement.
The patient, or Responsible Party where applicable, is responsible for all fees associated with services provided by MBR Health. At the time of booking, MBR Health will make reasonable efforts to provide an accurate estimate of fees and any applicable Medicare rebate or third-party funding entitlement; however, such estimates are provided as a guide only and are not guaranteed. If a claim is rejected, reduced, or unpaid for any reason, any outstanding balance remains the responsibility of the patient or Responsible Party.
Refunds will not be provided solely because imaging findings are normal, negative, inconclusive, or do not explain symptoms, further investigations or treatment are recommended, or a procedure provides limited benefit or does not achieve the desired outcome. Differences of clinical opinion do not, by themselves, indicate that a service performed warrants a refund.
Cancellation and non-attendance fees may apply in accordance with MBR Health’s Cancellation Policy in effect at the time of booking. Further information is available: mbrhealth.com.au/terms-of-use. Nothing in this agreement excludes any rights that cannot be excluded under the Australian Consumer Law.

4. Enduring Agreement
This Financial Agreement applies to the current service and any future services provided by MBR Health unless withdrawn by the patient or Responsible Party. Withdrawal does not affect fees, charges, or obligations arising from services already provided or appointments already booked.
